There are two different types of dates for the practice case: the date the forms are signed and the date of onset of disability.

For the dates the forms are signed, you can use the general dates in the progress notes of when the forms were completed with the applicant. That said, no need to worry too much about the exact accuracy of these dates when completing the SSA forms for the course.

For the date of onset of disability, we recommend using the applicant’s last date of significant work (for adult applications) or the date the condition(s) became severe enough to impact functioning (for child applications) and being consistent with the date that you identify across all forms.
